Middletown Community Health and Dental Center provides the following health services:

All visits require an appointment.

Be sure to arrive 25 minutes early to your appointment to allow time to fill out paperwork.

Patients without insurance coverage should also bring proof of income to their first visit (pay stubs, etc.).

In case of an emergency, do not wait for an appointment. Call 911 immediately. Examples of a health emergency include chest pain, extreme shortness of breath, vomiting of blood, and other serious abnormal symptoms.

Co-pays are due in full at the time of your visit.

As a service to you, they will bill your health insurance company (if insurance coverage is confirmed). Co-payments and deductibles are due at the time of your visit.

Accept cash, check, and credit card payments. If you are unable to pay for your care, please tell the receptionist. Will work on a payment plan with you.
