Monday-Friday: 8AM-5PM.

Sliding Fee Program:

At all BMRHC medical clinics (Marshall, Clinton, Fairfield Bay, Deer, Yellville, Green Forest and Huntsville, Arkansas), you may be eligible for the sliding fee program if you do not have health insurance or you have a co-pay on your insurance.

To find out if you qualify, you must bring:


  • Your most recent tax return (required annually)

  • DHS verification of income/food stamps, workers compensation, alimony, child support, or unemployment (required monthly)

  • Current verification letter of social security or pensions (required annually)



Qualifying participants may receive healthcare services for as low as $25 per visit (Sliding Fee B).

Lab work from another doctor or clinic, as well as drug screens not ordered by the primary care doctor are not included in the sliding fee program.

These charges are self-pay.

Patients in Madison, Newton and Carroll County will be referred to the dental clinic in our Huntsville location.
