Family Dental Services
Family Health Centers cares for patients' dental and oral health care.
Provide a wide range of dental services at FHC-Portland and FHC- Phoenix Health Center for the Homeless.
The dental team provides the following services to patients:
· Preventive care such as teeth cleanings and fluoride treatments
· Emergency dental services, including extractions
· Diagnosis and treatment
· Restorative treatment
· Oral health education
In order to become a Family Health Centers dental patient, you must first become an established patient with a FHC primary care provider.
Once you become a primary care patient at any Family Health Centers site, you may contact the dental office to schedule your appointment.
FHC-Portland Dental Office Hours:
Monday - Friday 8:00AM - 4:00PM.
Evening appointments are available from 4:30PM -8:30PM on some weekdays (days vary week to week).

1. How am I expected to pay?
2. What are the costs for services. Is this discounted or based on income?
3. What Documents am I required to provide?
4. Do you need me to provide income statements in order to verify discounts, mail to provide residency?
5. How are the instruments sterilized?
Heat sterilization is very important for all instruments. If you are not comfortable with their answer, you can choose another office.
