Good Samaritan Health Clinic provides the following health services: Vision Service Medical Service Dental Services: routine dental exams, x-rays, extractions. Dental patients are initially seen in the Clinic by the Nurse Practitioner and then placed on the schedule of one of the dental providers in his/her practice to receive dental care. To become of patient of Good Samaritan Health Clinic, you must be: Resident of Cullman County, Have two forms of identification Provide proof of all household income. Not be covered by Medicare, Medicaid, Veterans or private insurance. All applicants must provide a Medicaid denial letter.
