The Community Health Center affords access to medical services for those individuals who may be challenged with little or no income or lack of insurance.

By providing affordable care to the families in the community, this program eliminates a common barrier of limited access to care.

The facility offers comprehensive health services including: prescription drug assistance, physical exams, diagnostic imaging services and gynecological exams.

Also offer pregnancy care including: pre-natal vitamins, ultrasounds, childbirth care and care during postpartum period.

The Community Health Center is supported by Indiana University Health La Porte Hospital and grants through The Foundation and other supporters.

To be eligible for services, individuals and families must reside in La Porte County. (Michigan City residents are not eligible for Community Health Center services but may be eligible for services provided by HealthLinc Community Health Center).

All Community Health Center patients will be evaluated to determine their service qualification.

Qualification and financial eligibility is based on household size and income.
