Mission Statement:

To provide quality, affordable dental care to Livingston County residents with limited finances and little to no access to dental care while upholding the professional standards of dentistry in a concerned and compassionate way.

The VINA Community Dental Center of Livingston County, MI, a non-profit outreach facility, will give people access to dental care and dental education. Uniting people in the area, VINA will improve quality of life through better oral health, promoting dental care seeking behaviors and elevating prevention awareness. By providing dental education materials, oral cancer screenings, and promoting balanced nutrition and healthy food selections, they believe they can impact patients oral health and in turn, their overall systemic health.

Eligibility:

Patients must meet ALL 4 requirements:

1.Individuals must live in Livingston County
2.Individuals must be 19 years or older unless they are an emancipated minor
3.Households must have NO dental insurance
4.Households must have an income at or below 200% of federal poverty level. This also includes public assistance, social security, and unemployment.

Services Provided:

- Exams including oral cancer
- Cleanings
- Fillings
- X-rays
- Fluoride Treatments
- Extractions
- Dental Education Materials

Services NOT Provided:

- Braces
- Whitening
- Cosmetic Dentistry
- Sedation IV or oral
- Nitrous Oxide (Laughing Gas)

Applications Procedures:
- Please check our Recent News for updates on upcoming application opening dates.
- Patients must complete the following forms before being given an appointment. 1 2 3 4
- Patients will be required to pay a scheduling fee of $15.00 for each visit with either cash, money order, or certified check.
- All appointments must be PREPAID.
- Patients must bring proof of residence in Livingston County (ie: utility bill, etc.).
- Patients must bring proof of family income (ie: last years tax return, last three pay stubs, unemployment statement, or social security statement).
- Patients must bring a photo ID.

Important Information:

- Bring proof of household income* and residence every 6 months. \"Household Income\" is based per address, including anyone living at this address.
- Patients must arrive 15 minutes BEFORE every appointment.
- If patient misses an appointment, co-pay will not be reimbursed and patient will be put at the END of the wait list.

Children:

- Children in the waiting area must be supervised at all times.

Submission:

You may obtain an application at VINA Community Dental Center Monday-Friday from 9:00am-12:00pm and 1:00pm-4pm. Applications will be accepted on eligibility dates only, Future eligibility dates will be posted on our website.
