Do you perform extractions ? I lost a crown. Now the exposed decayed tooth is falling apart and leaving a hole causing infection and damaging by inside check. I need help ASAP. Please advise.

I have the parkland PHP insurance and just left the hospital after being in for 2 months.
It all started with a tooth I tried pulling and it apparently became assessed but I didn’t go to the doc until months later - it caused major heart issues, bleeding on the brain and slight kidney and spleen issues. I’m finally recovering and have been discharged but am still on a slew of medications but need the tooth worked on.
Here are my main questions/concerns:
1- will my parkland PHP insurance cover a dental implant? If not an implant, what about a bridge? It covered my 2 month hospital stay 100% but I’m unsure of all the plans benefits/coverages. The only steps needed are the post and tooth - the hospital took care of cleaning out the infection and remaining root.
2- how soon after getting released/discharged from the hospital will I be able to have dental work done?
3- what info is needed from me to go ahead and get an appointment set up to start the process.
